You can use the blank forms with the certificates at the bottom, or you can use the wording on your own documents. Affidavits are pursuant to Florida Statutes.

FL General Affidavit (Jurat) - 1 Signer

FL Acknowledgment - 1 Signer
Print out this blank acknowledgement, handwrite or type your information and have your signature notarized. For a single signer. [F.S. §695.25(1), 117.05(13)(b)]
FL General Affidavit (Jurat) - 2 Signers
Print out this general affidavit, handwrite your sworn statement(s) and have both signatures notarized. For two signers. [F.S. §117.05(13)(a)]
FL Acknowledgment - 2 Signers

FL Jurat - By Mark
This loose jurat certificate is sufficient for the purpose of notarizing for a person who signs with a mark. [F.S. §117.05(14)(c)(1)]
FL Acknowledgment - By Mark
This loose acknowledgment certificate is sufficient for the purpose of notarizing for a person who signs with a mark. [F.S. §117.05(14)(c)(2)]
FL Jurat - By Notary
This loose jurat certificate is sufficient for the purpose of notarizing for a person with a disability who directs the notary to sign his or her name. [F.S. §117.05(14)(e)(1)]
FL Acknowledgment - By Notary
This loose certificate is sufficient for the purpose of notarizing for a person with a disability who directs the notary to sign his or her name. [F.S. §117.05(14)(e)(2)]
FL Jurat - 2 Credible Witnesses
Use this jurat in conjunction with the FL Affidavit - 2 Credible Witnesses for taking an oath from a person without a statutory ID. [F.S. 117.05(5)(b)1, 117.05(13)(a)]
FL Acknowledgment - 2 Credible Witnesses
Use this acknowledgment in conjunction with the FL Affidavit - 2 Credible Witnesses for taking an acknowlegment from a person without a statutory ID. [F.S. 117.05(5)(b)1, 117.05(13)(b)]
FL Affidavit - 2 Credible Witnesses
This loose affidavit is used when the notary does not personally know the signer whose signature requires notarization. [F.S. §117.05(5)(b)1]
FL Acknowledgment - Attorney in Fact
Print out this blank acknowledgment, handwrite your information and have the signature of the attorney in fact notarized. [F.S. §695.25(4)]
FL Attested Photocopy
The notary should print out this loose certificate for use when attesting to photocopies, which the notary must make or supervise. [F.S. §117.05(12)(a)]
FL Acknowledgment - Partnership
Print out this blank acknowledgment, handwrite your information and have your signature notarized as partner of a partnership. [F.S. §695.25(3)]
FL Affidavit of Attorney-In-Fact
This is a loose certificate for the principle of an attorney-in-fact as requested by third parties. [F.S. §709.08(4)(c)]
FL Acknowledgment - Representative Capacity
Print out this blank acknowledgment, handwrite your information and have your signature notarized in your representative capacity. [F.S. §117.05(13)(c), §695.25(2)]
FL Physician's Affidavit
Affidavit made by primary care physician that a principle lacks the capacity to manage property as defined in F.S. 744.102(12)(a). [F.S. §709.08(4)(d)]
FL Acknowledgment - Public Officer, Trustee, Personal Rep
Print out this blank acknowledgment, handwrite your information and have the signature of the public officer, trustee or personal representative notarized. [F.S. §695.25(5)]
FL Translation Affidavit
Affidavit on which the signature of a translator can be notarized where the translator certifies and swears to the accuracy of his or her translation. [FL Gov's Ref Man for Notaries 2001, p64]
FL Affidavit of Confiscation of Identification
Affidavit for Law Enforcement Officers to attest to the confiscation of an inmate ID pursuant to FL Statutes. [F.S. §117.05(5)(b)2.h]
FL Passport Affidavit & Attested Photocopy
Used when one needs an attested photocopy made of their U.S. passport. [F.S. §117.05(13)(a), §117.05(12)(a)]
FL Self Proof Affidavit
For use with Last Will and Testament. Consult your attorney for relevant application. [F.S. §732.503]
